After you have booked your appointment through either our Online Booking System or by telephone, please take a few moments to read through our information (click the red links below) on New Patients:
INTAKE FORMS: You will receive an email link once you have scheduled your appointment, to complete your online intake forms.
GENERAL INFORMATION IN PREPARATION FOR YOUR FIRST VISIT:
- Our intakes have gone paperless! Your Intake form links will have be emailed to you after you booked your appointment either online or over the telephone. Please ensure that you have completed all forms and sent them back to our office well prior to your appointment day. If these are not completed by your scheduled appointment time, your appointment will be cancelled and may be rescheduled.
- If you are receiving Acupuncture on your Initial Visit, please come dressed in comfortable loose clothing eg sleeves or pant legs that can be rolled, without makeup, lotions, or fragrances
- For Naturopathic Visits, please bring copies of any lab work / blood work that you have had done via your MD, as these will expedite your time with Dr. Gill.
